Terms & Conditions :
- Test done outside the hospital or outsourced will be charged on actuals.
- Senior Citizen need to get themselves registered by filling up the application form available at the OPD counter.
- Registration fee needs to be paid (one-time).
- Free Lifetime membership after 5 continuous renewal.
- Privileges may change at the discretion of the Hospital Management.
- Privileges are applicable for a year from the date of availing the membership at the hospital counter.
- Benefits to be extended only after display of valid membership card.
- Membership fee is non- refundable.
- Golden Age discounts & benefits cannot be clubbed with any other scheme or capacity like TPA/ Corporate.
- Please collect your card within 45 days of becoming member.
Membership Fees :
- Membership for first year : Rs. 500/-
- Renewal fee per year : Rs. 500/- (Free lifetime membership of 5 continues renewal)
Registration Requirement :
- Proof (Voter ID / Aadhaar Card / Driving Licence / Passport
- 2 copy of passport size photograph
- Blood group proof
Eligibility Criteria :
Any Indian citizen above 60 years can become a member and avail the privileges offered under this scheme.
